When to Use the Charter School Complaint Form
This form is specifically for reporting violations of California Education Code Section 47605(e)(4). Unlike other forms, this complaint is submitted directly to the Solano County Office of Education rather than the school itself.
Use this form only if you believe the charter school has:
Discouraged Enrollment: Attempted to prevent a student from enrolling based on academic performance, disability, English learner status, or other characteristics.
Required Records Prematurely: Requested or required a student’s records before they were officially enrolled.
Encouraged Disenrollment: Pressured a current student to leave the school or transfer elsewhere for any reason other than suspension or expulsion.

When to Use the General Complaint Form
Use the General Complaint Form for formal concerns regarding the conduct of school personnel or specific school site issues that do not fall under federal or state program violations.
Use this form if you have a complaint about:
Employee Conduct: Issues or disagreements involving a teacher, administrator, or staff member.
School Site Operations: Concerns regarding day-to-day school procedures or environment.
Other General Issues: Any formal concern that is not related to Title IX (sexual harassment) or UCP (state/federal program violations).

UCP Complaint Form
Is the Uniform Complaint Procedure (UCP) Form right for you?
Use the UCP Complaint Form if you are reporting a formal violation of state or federal law regarding:
Discrimination, Harassment, or Bullying: Based on race, disability, religion, age, or other protected characteristics.
Specialized Programs: Violations related to Special Education, English Learner programs, or Career Technical Education.
Illegal Student Fees: Being charged for a class or activity that should be free.
LCAP: Issues with the school’s Local Control and Accountability Plan.

Title IX Complaint
When to Use the Title IX Complaint Form
The Title IX Complaint Form is used specifically for reporting incidents of sexual harassment or discrimination based on sex in any educational program or activity. These complaints follow a specific federal grievance process.
Use this form if you are reporting:
Sexual Harassment: Unwelcome conduct of a sexual nature that is so severe, pervasive, and objectively offensive that it denies a student equal access to education.
Sexual Misconduct: Including sexual assault, dating violence, domestic violence, or stalking.
Quid Pro Quo Harassment: When a school employee conditions an educational benefit or or participation on an individual’s participation in unwelcome sexual conduct.
Gender Equity Issues: Discrimination based on pregnancy, childbirth, or gender identity, including inequity in athletic programs.
